SEND Survey: Planning the future of SEND

Dear Parent / Carer

We want to know what life is like in Northumberland if you have a child with a special educational need or disability (SEND).

This survey has been developed with Northumberland Parent Carer Forum and a group of parents and carers who are working with Northumberland County Council and NHS commissioners.

We would be grateful if you would fill in this survey. 

If you have more than one child, you may complete the survey for each child, or include your thoughts about your family on one form.

Following the survey, we will write a report telling you what we have found out and what we will do in response to the feedback.
