Year 6 and 7 Students in Berwick middles

Overview

Parents of Year 6 and 7 students in Berwick Middle Schools are asked to complete the attached form to confirm either:

1) You wish to take up the guaranteed place for your child as named in this questionnaire and therefore agree to the release of their student file to Berwick Academy.

2) You do not wish to take up the guaranteed place for your child and therefore their student file will not be released to Berwick Academy.

If you have responded No to Question 7, please refer to our previous guidance dated 2nd March regarding in-year applications for other schools.
